There is no limit or cap. Leech is always a heal-over-time, unless you have Vaal Pact.

Example, you leech 500 Life, you have 500 Life.
20% of 500 is 100, so you leech at 100 hp per second. Five seconds of 100 hp/s.
500 Life leeched, 10000 Life.
20% of 10k is 2000. At a rate of 2000 per second, you regenerate 500 Life in 0.25 seconds.

Either way, it's a Heal over Time that equates to 20% Max Life per Second.

When you max out your Life, any leftover Leeched Life is discarded. This means it's not efficient to Leech way more than you need to hit 20% Max Life per Second. Odds are, it's just going to be discarded anyways. It's not a literal cap, but an effective cap.

The Quality bonus on Leech gems improves the speed at which Leech applies. With a 10% Quality Gem, your Leech Rate is +10%. Any Life leeched with that gem regenerates you at 22% Max Life per Second, as opposed to 20%/s.
