Weapon Set bug on Console

I am experiencing a very strange issue where my wand & focus do not show on play character when equipped to weapon slot 1.

I am only able to use the wand and focus when they are slotted to weapon slot 2. My sprint auras can only be toggled on weapon slot 2, removing and equipping skill gems seems to not help or only momentarily fix the issue. And lastly the stats from the equipped wand and focus only apply to the player character when they are slotted to weapon set 2.

The issue seemingly began after I swapped wands hoping to upgrade and now I am having trouble playing on the character.
