Missing Item After TP-ing out of Map

Recently purchased a Coming Calamity, used it without any problems. When suddenly my chestpiece dissapeared! I was switching around some spirit support gems right before i TP-ed out of the map! After reloging the item returned in my inventory, although all the support gems went missing, also changes made to the socketables returned to it's original runes (after purchasing the item I swapped out some runes, it reverted back).
