How to improve this Doryani's Fist build (Standard)?

I'm pretty new to the game and this is the farthest into the endgame I've gotten and would appreciate some advice.

With blood stance up and swapping out Pulverize and Critical Strikes for Ruthless and Conc Effect I can get to about ~4m dps with flasks. The main problem I'm having is survivability against end game bosses and some t16 map bosses.

Any suggestions on how you would improve this build
